A technique that I have used (first gleaned from a John Shaw book)
is to mount a TTL flash (using a flat bar, two 1/4" tripod screws,
a cheap mini ball head, and a TTL remote cord) with the end of the
flash at the front edge of the lens, at the "1:30" clock position
(looking from the front - good then for both H and V camera
orientations), pointed directly at the subject. Lighting from the
relatively large flash tube (compared with the small subject)
produces a nice, soft light from a nice overhead location- and the
flash power allows very small effective apertures (with small stops
combined with the macro extension, and when using additional tubes
and/or tele-converters) to be used for good DOF and ease of
hand-holding. This rig makes high-magnification photography almost
easy, and the results are excellent.
