This may sound odd, but the best way may be by shooting them
with your video camera directly. I've often found the results
cleaner than trying to bring scans into Premiere.
If you don't get things level, try using the "clip" filter
to produce straight borders (in any color). I use a Nikon
52mm-thread slide-holder, about $50 (sometimes with one
or two achromats added to get the size and focus right),
but you can probably rig up a useable slide-holder out of
cardboard, with a simple light diffuser added - and most
camcorders near the short end of the zoom range will focus
on very near objects...
