This can happen when you adjust the audio track after
rendering the video transition (it's a bug). Move the
transition to destroy the preview, put it back and
re-render... In general, I regard the previews while
editing as just that - and when editing is complete,
I go to the preview file folder, delete all the previews,
then build them all at once before outputting to tape.
